I understand that my puppy has to be vaccinated with all age appropriate vaccines, including Bordetella, prior to the start of puppy classes. These vaccines also need to have been administered by a Veterinarian.
Terms and Conditions for Puppy Socialization Class

Please read the following statements carefully
I understand that this class is designed for puppies between 8-16 weeks old.
I understand that I must provide updated vaccine and deworming records at least 7 days prior to the start of puppy classes in order to attend.
I understand that if my puppy is not feeling well prior to the start of each class, I will update Brintnell Veterinary Hospital or the Trainer and inform them. Symptoms include: Vomiting, diarrhea, lethargy, and coughing.
I understand that the trainers will only utilize positive reinforcement and scientifically proven training theories in attempt to offer safe, sound, and responsible training and training instructions. Use of positive punishment items, including but not limited to choke chains, prong/pinch collars, or remote trainers is strictly prohibited. Introduction of these items will result in termination of this training contract. NO REFUNDS of any paid amount will be given.
The trainer will endeavor to create as safe of an environment as possible for the training of my animal. I understand that their are risks when signing up for group classes and animals sometimes have unpredictable reactions and behaviors. Further, I understand that although the trainers are providing a safe environment, there are risks for environmental hazards and unforeseen circumstances. I agree to hold the aforementioned parties harmless from any claim for the loss, injury, illness, damage or otherwise of my pet that may occur as a result of these puppy classes.
I understand that if I want to cancel my registration and receive a refund, that I must provide written notice 7 days prior to the start of classes. I understand that refunds will not be provided for classes after the session starts.
