By signing this agreement, I, the undersigned, acknowledge and agree to the following terms and conditions in order to participate in activities at Dreamland Play Space and Cafe:
1. Assumption of Risk I understand that participation in activities at Dreamland Play Space and Cafe may involve inherent risks of injury, illness, and accidents. I voluntarily assume all risks associated with participation in these activities, including but not limited to physical activities, use of equipment, and interaction with other participants.
2. Release of Liability I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby fully and forever release and discharge Dreamland Play Space and Cafe, its owners, employees, agents, and affiliates from any and all claims, liabilities, demands, actions, or causes of action that may arise from personal injury, death, property damage, or any other loss or harm that may occur during participation in any activity or while on the premises, whether caused by the negligence of Dreamland Play Space and Cafe or otherwise.
3. Indemnification I agree to indemnify and hold harmless Dreamland Play Space and Cafe, its owners, employees, agents, and affiliates from any and all claims, damages, or expenses (including attorneys' fees) arising from my actions or the actions of any minor child under my care during our visit.
4. Medical Treatment In the event of an emergency, I consent to any medical treatment or emergency care that may be required during my participation or the participation of any minor child under my care. I understand that Dreamland Play Space and Cafe does not provide medical insurance and that I am responsible for any medical expenses incurred.
5. Parental/Guardian Consent If signing on behalf of a minor child, I hereby give my consent for the minor to participate in activities at Dreamland Play Space and Cafe. I acknowledge that I am responsible for the supervision and safety of the minor while on the premises, and I agree to ensure that the child follows all safety rules and instructions provided by Dreamland Play Space and Cafe.
6. Rules and Regulations I agree to abide by all rules and regulations set forth by Dreamland Play Space and Cafe, including safety rules, posted guidelines, and any instructions provided by staff. I understand that failure to comply with these rules may result in removal from the premises without refund.
7. Photographic Release I give Dreamland Play Space and Cafe permission to take photographs or videos of me and/or my children during our visit. These images may be used for marketing purposes, social media, or other promotional materials. I waive any right to compensation for such use.
8. Severability If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
9. Governing Law This waiver shall be governed by and construed in accordance with the laws of the jurisdiction in which Dreamland Play Space and Cafe operates.
By signing below, I acknowledge that I have read, understand, and agree to the terms and conditions of this waiver and release of liability. I further acknowledge that I am voluntarily agreeing to these terms and that I am signing this waiver of my own free will.
