Nail Salon Consent Form

I acknowledge that the nail technician is a licensed professional and should be treated with respect all the time.I confirm that to my knowledge I am free of any fungal infection and if my nail technician notifies me that i may have one, the service may not be continued and i will seek medical attention.I am allowing the nail salon and the nail technician to apply necessary chemicals as part of the service in my nail treatment.I agree that the nail service is final after the service. If there are any changes after the service ends, the client will be charged.I understand that kids are not allowed in the work service area for safety reasons.I have read this whole document and I accept the terms indicated above.I acknowledge that the nail technician has the right to refuse service for any reason.I consent to paying the full amount due at the end of my service.I acknowledge that my nail technician may not be able to give an exact desired outcome but try her best to satisfy my vision.
