  • EMERGENCY INFORMATION

  • Should my child become ill or suffer an accident, I hereby authorize Imagine Nation Learning Center, to call for, or secure the necessary emergency care of medical attention as deemed necessary by Imagine Nation Learning Center. I understand that an effort will be made to contact myself or the designated persons if possible, before any action is taken. I also understand that any expense incurred will be accepted by me.

  • List an individual OTHER than parents/guardians who may be contacted to pick up your child in case of an emergency.

  • *If the child does not live with both parents, a copy of the court-ordered custody decree must be maintained in the child's permanent file. The guidelines of the custody decree will be strictly enforced.

  • AUTHORIZED "PICK-UP" INFORMATION:

  • SUMMER CAMP - REGISTRATION AND FEES

  • FEES

    • Registration Fee: $50.00 (non-refundable) – Due at the time of enrollment. (Applies to both existing and new families)

    • Pirate's Cove Fun Zone Pass: $65.00 (one-time) – Due at the time of enrollment.

    • Field Trip Fee: $40.00 per month – Due on the 1st Monday of each month.

    • Tuition: $185.00 per week – Due by close of business on Monday each week.

    PAYMENT OPTIONS


    Upon reviewing your enrollment application, a director will contact you soon to arrange payment for the registration fee and Pirate's Cove Fun Zone pass fee. Once payment is received, your child's enrollment will be finalized. We accept payments via automatic bank draft, cash, or credit card.

    TUITION

    Tuition is due by close of business on Monday each week. If payment is received after 6:30 PM Monday, I will lose any discounts that may apply and a late payment fee of $30.00 will be applied. Failure to stay current in your child's tuition fees may result in the loss of your child's spot in our program and will constitute a default which will entitle our school to all remedies as prescribed by law including reasonable attorney's fees. My child's tuition rate is $185.00 per week for the Summer Camp program plus $40.00 per month for the cost of field trips. I understand that rates are subject to change with reasonable notice as conditions require. If I choose to change my enrolled program, I will be required to complete a new Enrollment Agreement. There are no deductions for holidays or partial week attendance. 

    LATE PICK-UP CHARGES

    The center is open from 6:00 am to 6:30 pm Monday through Friday all year, except holidays listed in the Family Handbook. If a child is left after closing, an administrator will attempt to contact the parents/guardians first, then will proceed to the listed emergency contacts to pick up the child. I understand that I will be charged a late pick up fee of $15.00 for every 15 minutes or portion of a 15-minute period, per child, until the child is picked up. This fee must be paid at the time of pickup.

    WITHDRAWAL

    Should it become necessary to withdraw your child for any reason, be sure you have reviewed the withdrawal procedures listed in the Parent Handbook. A two-week notice is required when withdrawing. Verbal notice will not be considered a withdrawal notification. A charge of up to two weeks will be incurred for improper notification. 

    RETURNED ACH PAYMENTS

    I understand I will be charged a fee of $33.00 if my check/ACH payment is returned for non-sufficient funds and my ach payment will be resubmitted electronically up to three times. If more than two checks/ach payments are returned within a calendar year, I will be required to make future payments by money order, credit card or cash. 

    VACATION CREDITS

    We encourage families to take advantage of Vacation Credits if your child is going to be out for an entire week and you have been enrolled more than a year and have not already used your vacation credit. Vacation Credits reduce your weekly tuition payment by 100%. The Vacation Credits must be taken in full-week increments. There is no credit given for single days. Regular tuition must be paid when your child attends any part of the week. Vacation Credit requests must be submitted in writing to the Center Director two weeks prior to use. Each family will receive one vacation credit per year, per child. See Center Director for vacation credit details. 

    ILLNESS

    I understand that if my child is ill, including but not limited to: a severe cough or sore throat, undetermined rash or spots, temperature (see parent handbook for details), severe headaches, upset stomach, pink eye or diarrhea, he/she cannot be accepted into the center until well. Children must be fever free for 24 hours (without fever reducing medications) before returning to the center. In the event my child has a communicable disease, a release from a medical source may be required before my child re-enters the center. Imagine Nation will notify me if a reportable disease has been introducted into the center. 

    MEDICATION

    Medication will only be administered once a day and all medications must be signed into the front desk. NO MEDICATION MAY BE PLACED IN THE CHILD'S BAG. Only medication specifically labeled as a prescription with doctor's name, child's name and dosage procedures will be administered. The center will only administer over the counter medications according to the directions on the label. When the directions indicate "ask doctor," the school requires written authorization from the child's physician. Benadryl or its generic form will not be administered.

    SIGNING IN AND OUT

    I understand that it is my responsibility to escort my child in and out of the school as well as sign my child in and out of the center. I understand that staff members will escort my child to and from the center when being transported via Imagine Nation transportation to and from field trips.

  • PHOTOGRAPHY/VIDEO/SOCIAL MEDIA RELEASE

    I authorize permission to Imagine Nation Learning Center, its agents, affiliates and licensees to make, reproduce, publish and otherwise use photographs, videos, and/or sound recordings of my child for the purpose of illustration, advertising, and publicity, in any manner or in any form, including broadcast, print, electronic and social media. I agree to indemnify, defend and hold harmless Imagine Nation Learning Center, its agents, affiliates, licensees and employees from claims arising from or relating to the use of the video/photograph or sound recordings of my child and hereby waive release, and discharge any claims I may have against Imagine Nation Learning Center, its agents, affiliates, licensees and employees arising from such use. I understand that my child will not be identified without written consent. I understand that this approval may be revoked at any time by written request to the management of the school my child attends.

  • 1. I understand that I am enrolling my child in Imagine Nation Learning Center's Summer Day Camp Program. Hours of operation are 6:00 a.m. to 6:30 p.m. - Monday thru Friday.

    2. I understand that I am responsible for the following fees:

    3. I understand that this Summer Camp Program will be for approximately 11 weeks from May 27 thru August 8, 2025. (Closed July 4th for Independence Day)

    4. I understand that on field trip days, my child must arrive no later than 8:00 a.m. to be included in the field trip, as buses are on a tight departure schedule. Any child(ren) arriving after 8:00 a.m. may remain at the center and be placed with a teacher for the day to participate in in-class activities. 

    5. I understand that on field trip days, I am responsible for providing a disposable sack lunch (PLEASE DO NOT INCLUDE ANY PEANUT BUTTER OR OTHER FOODS/SNACKS THAT CONTAIN NUTS). 

    6. I understand that if my child displays unsafe behavior and/or behavior disrespectful to camp staff and/or other campers, I will be advised of such behavior and asked to discuss said behavior with my child. If the behavior isn't modified and continues, Imagine Nation will advise the parent of the continued behavior and the child may be placed on field trip suspension for up to 4 field trips. I understand that during this suspension period, I will still be responsible for paying full tuition and monthly field trip fees. If the behavior continues or is at a level that is determined to be a threat to the safety of staff, other campers and/or the child him/herself, Imagine Nation reserves the right to disenroll the child from Summer Camp. If a child is disenrolled, no refund will be issued for the enrolled period. 

    7. I understand that in the event of any absences during Program hours activities, I will be responsible for fees for time reserved, not actual time spent at the Program (no per diem for care). Should my child be absent during a day, or several days, the fee is still the same for the entire week.

    8. I further understand that I am responsible for notifying Imagine Nation Learning Center ahead of time when my child WILL NOT be attending the program. Voice mail is available to leave a message. I will give a two (2) week notice in writing prior in case of early withdrawal from the program during which time I will be responsible for payment of fees.

    9. The Program staff will assume full responsibility, as deemed reasonable, for my child from the time he/she arrives at the Program until my child leaves the Program.

    10. If a medical emergency arises, the Program staff will first attempt to contact me. If I cannot be reached, the staff will contact the emergency contact I have specified on the enrollment form. If the emergency is such that immediate hospital attention is necessary, an ambulance or emergency vehicle may take my child to the hospital.

    I agree to adhere to the Policies and Procedures of Imagine Nation Learning Center's Summer Day Camp as stated here and therefore give my child permission to participate fully in this Program.

  • IMAGINE NATION LEARNING CENTER SUMMER DAY CAMP PROGRAM RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT

    This is a release. Read it carefully before signing. By signing this release, you are giving up your and your child's rights to sue Imagine Nation Learning Center, its agents, officers, volunteers, employees and any parties that operate, administer, co-organize or provide transportation to or from the activities described below (collectively, the "Released Parties") or expect the Released Parties to be legally responsible or pay for any damages or medical expenses if your child is injured or killed, becomes ill or your child's belongings are damaged as a result of your child's participation in the activities described below.

    I acknowledge that I have voluntarily chosen for my child(ren) to:

    (a) participate in IMAGINE NATION LEARNING CENTER SUMMER DAY CAMP, a day program administered by the Released Parties, for summer day childcare, which may include numerous activities, including, but not limited to, sports, hikes, arts and crafts, science experiments, cooking projects, and field trips to various locations by bus, any activities incidental thereto and

    (b) be present at or use, as applicable, facilities, other locations, equipment and/or transportation provided by the Released Parties or others in connection with my participation in such activities

    (the activities in clauses (a) and (b) are referred to collectively as the "Activity"

    Acknowledgment and Acceptance of Risks.

    I understand that certain risks are inherent in the Activity, and that these risks cannot be eliminated, altered or controlled. I understand that the risks that contribute to the unique character of the Activity can also be the cause of my child(ren)'s injury, illness or death or damage to my child(ren)'s belongings. I voluntarily elect, with knowledge of the risks involved, for my child(ren) to participate in the Activity. I acknowledge and willingly assume all risks and hazards in my child(ren)'s participation in the Activity and in the use of the Released Parties' facilities and/or equipment.

    Release.

    I am the parent or legal guardian of the child(ren) listed in the enrollement agreement provided. In consideration for my child(ren) being permitted to participate in the Activity, I voluntarily agree and promise not to make a claim against, sue or attach the property of the Released Parties, and I release, waive, discharge and hold harmless the Released Parties for all demands, actions or claims of liability arising out of their negligence, fault, recklessness or any other act or omission that causes my child(ren)'s illness, injury, death and/or damage to me or my child(ren)'s property as a result of my child(ren)'s participation in the Activity and in the use of the Released Parties' facilities and/or equipment.

    Knowing and Voluntary Execution. I have read this document in its entirety. I understand that by signing this document, my child(ren) and I are assuming all the risks of the Activity. I understand that this is a release of any and all claims. I understand that this is the entire agreement between us and the Released Parties and that it cannot be modified or changed in any way by oral statements by any Released Parties or by us. I voluntarily sign my name as evidence of the acceptance by me and my child(ren) of all the provisions in this document and our agreement to be bound by them.

  • Camp 2025 Sunscreen Permission Slip

    Ensuring a safe and fun summer camp for our staff and campers is our ultimate priority. In an effort to decrease confusion, ensure staff and camper safety and improve current processes, we have decided to provide sunscreen during water play field trips for all of our campers. This will be at no additional cost to you; however, we will need your permission to apply the following sunscreen brand and SPF level to your child throughout the summer.

  • If 'No', I understand that I will be responsible for providing sunscreen for the duration of the summer at my own cost. 

    I hereby grant permission for Imagine Nation Learning Center staff to apply the sunscreen I provide to my child.
