Mountain View Summer Camp 2025

I hereby grant permission for participant(s)to participate in Mountain View  Play School as they are able bodied to do so. By signing this I acknowledge that I have been granted the authority to inspect the grounds and recognize that the land is in a natural state, undeveloped and wild in the surrounding areas and contains hazards and risks that could cause injury or death. I hereby release the staff of  Mountain View Play School (213 Mountain View rd.) from all claims in illness, injuries or other damage including death which may be sustained by the child during attendance at Mountain View Play School.  I furthermore agree that I will not sue or hold any of the above mentioned parties liable in this respect.  I recognize and grant permission to the staff of Mountain View Play School to make every effort to care for my child’s health and well being in the best way they can at all possible times. In consideration of the applicants being allowed to participate,  we hereby authorize the staff members of Mountain View Play School to obtain assistance from doctors and nurses at the closest hospital for the above mentioned child. Furthermore, I grant permission and consent for the attending physician to provide any medical assistance deemed  necessary if the issue is life threatening to my child. I understand that I am responsible for medical, dental, legal, or any  other emergency expenses incurred by my child. This document serves for the entire time my child is enrolled at Mountain View Play School and on the school property.In signing this I agree that if anything is deemed to feel unsafe by the caregiver, that the undersigned will advise the staff and has the right to leave and refuse to participate further in Mountain View Play School.

Each week of camp is $260 - there are no sibling discounts available at this time.
