AS ONE YOUTH CAMP Registration Form
This is a non contact football camp where youth players can come learn skills and play football in a fun competitive environment. Camp will run June 25th June 26th and June 27th from 9:00 AM to 12:00 PM. Registration is $175 per camper. Payment can be cash, check, or on stripe. It's encouraged that anyone participating wear cleats, but this is not mandatory.

I understand that the tuition to be paid is for the designated dates (Rain or Shine) and covers instruction, camp T-Shirt, insurance, and awards. No deduction shall be allowed for early departures or late arrivals. I herby certify that my son is in good health and may participate in all camp activities. In case of emergency I grant permission for my son to be given treatment at a local hospital.
