I understand my information and the information about my child(ren) will be processed by DCSF in accordance with its privacy notice.
Parental Consent
To be agreed by parent or guardian: I understand and hereby agree with the following statements:
(a) No liability shall arise or occur against DCSF for any loss, damage or theft of personal property belonging to said child, whilst participating in any DCSF activity.
(b) No liability shall arise or occur against DCSF for any accident, injury or death occurring whilst participating in DCSF activity, except to the extent that such injury or death was the result of a negligent act or omission of DCSF.
(c) All information provided is true and accurate and the above child is in good physical condition and capable of engaging in the level of exercise associated with activities. Exercise will not be detrimental to said child's health, safety, comfort or physical condition.
(d) If said child's health status/ medication changes whilst engaged with the project I shall notify staff immediately.
Media Consent
Occasionally, we may take photographs of the children during activities and events. We may use these images in printed media produced by DCSF and/or other stakeholders, such as prospectuses, guides, training products, magazines and newspapers, as well as on websites hosted by DCSF and other organisations including social media such as Facebook.
We may also make video or webcam recordings to promote the work of DCSF to partners, funders or other charitable institutions or for other educational use.
By agreeing to send your child on a DCSF you give us permission to use thier image for these purposes. If you would prefer to opt out of this please email events@dcsf.org.uk.
Conditions of Use
This form is valid for 3 years from the date you sign it. The consent will automatically expire after this time.
We will not re-use any photographs or recordings after the 3 year period. We will not use the personal details or full names (which means first name and surname) of any child in a photographic image, on video, on our website, in our prospectus or in any of our other printed publications.
We will not include personal e-mail or postal addresses, or telephone or fax numbers on video, on our website, in our prospectus or in other printed publications.
If we use photographs of individuals, we will not use the name of that child in the accompanying text or photo caption. If we name a child in the text, we will not use a photograph of that child to accompany the article. We may use group photographs or footage with very general labels such as Holiday Camps 2023.
Cancellation policy
If you book any days and then wish to cancel, a refund will only be provided if the camp is fully booked and has a waiting list where we can backfill the place with a child on the waiting list. This is to ensure we have sufficient staffing for all camps and to minimise late cancellations.
To cancel and request a refund please email Debbie Bennett at events@dcsf.org.uk who will try to fill the place in order for you to obtain a refund.
