  • Please complete one form per child and submit.  * Indicates required response.

  • •Payments are only accepted on-line using FACTS for OLP families, or via the church website for families from other schools (Coach Kyle will send you the payment process in August).

    • Invoices based on the proposed schedule will be sent out on or near the 1st of each Month, due 15 days after posting.

    • We continue to pay our staff on unexpected school closures such as snow days. Reimbursements or credits will not be applied unless the SACC program closes (staff sickness etc) while the school remains open. 

  • One day drop-in rate: $20 per child – SACC registration paperwork must be turned in prior to sending your child to SACC. Please get approval each time you in intend to drop in before 1:00 PM day of by emailing kdavis@olp-parish.org.

    New this year: All drop-ins must be picked up no later than 5:30 PM.

    When possible, notify me 2 weeks ahead of any schedule changes. Schedule changes requested within 2 weeks will not be credited. 

     

  • Our Lady of Peace SACC Permissions List

    The following permissions will remain valid for the entire school year or until withdrawn in writing.

     

    I acknowledge if someone other than the 2 guardians listed above is picking up my child/children, I will need to email the director with their first and last name, and I will alert the driver to have their ID available. Additionally, I may be required myself to present ID if it's the first time I have picked up my child from SACC. 

  • Movies – We will typicallywatch movies or shows during snack. They will typically be rated G or TV-Y.

  • *Note – A medication form must be completed for each medication administered while at the SACC program. This form is available in the school office or on the SACC page of the OLP school website.

  • LETTER OF AGREEMENT

  • 1. I will complete all necessary forms (medical, consent, etc before my child attends the SACC program.


    2. I will notify SACC of any changes in registration information.


    3. I understand I am responsible for paying program fees which must be submitted electronically. Failure to pay or a delinquent account greater than one month's dues could result in a pause or removal from the program. Please call Kyle Davis if you have any questions regarding payment.


    4. I understand if a child is not picked up by 5:59 PM I will be charged a fee of $20. Three or more recurrent late pickups are subject to removal from the program.


    5. Upon the arrival of the guardian, you/they are responsible for your child. This includes on the playground, re-entering the building, etc. Once you arrive, you must depart with your child or exit your vehicle and remain with your child.

     
    6. I will make other arrangements for emergency closings and early dismissals due to weather or facility emergencies. I understand that the SACC program at OLP will not be in session when Our Lady of Peace and Columbus Catholic Schools are closed. SACC will not be open when OLP makes an early release, including the final day of school. 


    7. I will inform SACC in writing if I do not want photographs of my child participating in the program to appear in any SACC media publications.


    8. I agree to read the Our Lady of Peace Parent/Student Handbook, which contains detailed information regarding the SACC program. I will also read the SACC update emails that may mention changes in policies, schedule, etc.


    9. I understand guardians typically should not enter the building. Guardians will be given a phone number to call when you arrive to pick up your child. This number will connect you directly to a SACC staff member. We will dismiss the child out the west door of the school. (Same as morning drop-off) 


    10. Anyone picking up your child for the first time must present a photo ID, and you will have emailed the program director to let them know. 


    11. I understand that I must inform the director each day/time that my child is scheduled to attend SACC but will not be present by email. I understand failing to do so will result in the program halting until confirmation of the child's whereabouts. I recognize the program is run by the Church, and will not always communicate with school staff about absences.
