By submitting this application, I understand that the decision to approve or deny a foster placement for my pet is at the discretion of Serenity Kennel. I confirm that all information provided about my pet, including health, behavior, and history, is accurate to the best of my knowledge. I acknowledge that providing false or misleading information may result in the denial of foster care services.

I agree to comply with all guidelines and policies set forth by Serenity Kennel throughout the foster process, and I understand that I am responsible for ensuring the pet’s well-being until it is placed in a temporary foster home. This includes maintaining up-to-date vaccinations, addressing any medical needs, and informing Serenity Kennel of any changes to my pet's health or behavior.

I understand that Serenity Kennel will contact my veterinarian to verify my pet's vaccination records and other relevant medical information.

I understand that for the safety and protection of myself and others involved in this program, I will not be permitted to visit my animal(s) while in the temporary assistance program. I acknowledge that this is a policy designed to prioritize the well-being of the pet and all parties involved.

I understand that Serenity Kennel will make reasonable efforts to find an appropriate foster home but cannot guarantee a foster placement. I also understand that Serenity Kennel will contact me when a foster home has been arranged.

I acknowledge that I am responsible for any veterinary care that is required while my pet is in foster care. The need for veterinary care will be determined at the sole discretion of Serenity Kennel, and I agree to cover any associated costs for such care.

I also agree that, if a foster home is found, I will cooperate fully with the foster caregiver and Serenity Kennel, and that the pet will be returned to me as agreed upon at the conclusion of the foster period.

By submitting this application, I consent to the collection and use of my personal information for the purpose of processing my request and facilitating the foster placement. I acknowledge that this application does not guarantee approval for foster care and is subject to review and availability of foster homes.
