MP Fit Waiver

Assumption of Risk, Release, and Indemnity Agreement

Participation in any method of exercise carries an assumption of risk, including but not limited to injury, accident, illness, or death, whether caused by the undersigned or someone else. By signing below, the undersigned acknowledges and voluntarily accepts this risk and agrees that Julissa Munsters, her successors, or any officer, director, member, agent, servant, or employee will not be liable for any injury or death, including but not limited to: Personal, bodily, or mental injury

This applies to the undersigned, their spouse, children, or relatives (hereinafter collectively "CLIENT") resulting from the CLIENT's use of the facilities, including as a result of the negligence of Julissa Munsters or anyone else using the facilities. If any claim arises from an injury, death, loss, or damage described herein involving the CLIENT, the undersigned agrees to: Defend Julissa Munsters against such claims and cover all expenses, including attorney's Indemnify Julissa Munsters for all obligations resulting from such claims.

This document shall be governed by the laws of the State of Michigan. Any legal action must be instituted in the courts of Eaton County, Michigan.

Waiver of Liability: The CLIENT agrees to release, discharge, and not take legal action against the following for any harm or damage incurred:

Julissa Munsters
Any other CLIENT, visitor, or person present or using the facilities
Designers, manufacturers, or installers of the facilities or equipment
Employees or independent contractors of Julissa Munsters
The landlord of any/all studios applicable

This waiver applies to CLIENT's use of any studio facilities, equipment, or participation in activities sponsored by Julissa Munsters, including but not limited to Pilates, whether these activities take place on or off the premises.

The CLIENT, their heirs, next of kin, assigns, or personal representatives waive any and all claims against Julissa Munsters and any studio used for losses, damages, or claims related to personal injuries, property damage, or death, even if caused by the negligence of Julissa Munsters or the studio's employees or volunteers.
