GROUP CLASS ENROLLMENT - IMPORTANT NOTES (PLEASE READ)

• TERM 1 2026 (April-June) group lessons will consist of 8 or 9 sessions depending on class day (no classes will run on public holiday days).
• PRO RATA: For students who cannot join selected dates due to holiday travel/plans - A pro-rata rate will be charged if you can notify us in advance through this form. 
• WEATHER: If a class is cancelled by the coach due to inclement weather, the coach will offer make up class options throughout the term. If no suitable options are offered then a credit to the following term may be applied (There will be no refund for classes cancelled due to weather).
• SICKNESS: Any student who misses a class due to sickness (Doctor's Note required) can arrange for a make up class (up to 2 times per term) during the current term by contacting the coach directly. Please note there will be NO REFUND and NO CREDIT for a missed class due to sickness.

• NON-MEMBER FEES: The fees listed below are for members of The Country Club at Hong Lok Yuen. Non-Member's will have approximately 20% added to lesson fees.

CLASS TERMS & CONDITIONS - Please read carefully

By enrolling in swim classes (group or private), participants agree to abide by these terms and conditions below.

ENROLLMENT:

1. Swimming lessons are for Club members and their guests only.
2. All members/guests must submit application form through JOTFORM prior to commencing their first lesson.
3. You will receive an email upon successful receipt of JOTFROM application. All applicants will then be contacted directly by the coach in order to confirm class timing. 

PAYMENT: 

1. For Club members, payment for the full term will be debited to their membership account (on a monthly basis) once lesson is confirmed.
2. For non-members, full course payment must be made after receiving lesson confirmation by bank transfer or by cheque. A 20% surcharge will apply to the total course fee. Bank transfer details: HSBC 808-271852-001 or if paid by cheque, made payable to Hong Lok Yuen Country Club Ltd. If payment is made by bank transfer, please e-mail the bank transaction receipt as payment proof to recreation@cchly.com.
3. Non-member payments cannot be made through a members account.
4. All payments must be made prior to the start of the first class. 

CLASS POLICIES:

1. Children under 10 should be accompanied by an adult and all children should be collected promptly after the lesson has finished to ensure their safety.       

2. There will be no group lessons during Public Holidays.   

3. Requests to change class days/times are subject to coach approval and availability.

4. NO SHOW: If a student does not show up for a scheduled lesson, the full lesson fee will still be charged and there will be no refund or make up offered.

5. SICKNESS: Any student who misses a class due to sickness (Doctor's Note required) can arrange for a make up class during the current term by contacting the coach directly. If you are unable to attend one of the make up classes offered please note that there will be no refund/credit offered and makeup sessions cannot be carried over to the following term. 

6. In order to run the swimming program smoothly, a maximum of 2 sick-leave makeup lesson applications are allowed per student per term. 

7. There will be no refund or credit offered if you fail to attend a scheduled makeup lesson. This will count as though the missed lesson has been made-up. 

8. Private lessons must be cancelled or rescheduled at least 48 hours in advance. Late cancellations will incur full session charges. 

INCLEMENT WEATHER POLICY:

1. Lessons may be shortened or cancelled in the event of thunderstorms, typhoon signal No. 8 or Red/Black Rain Storm warning being hoisted.
2. Students will be notified by the coach via Whatsapp/SMS/Email at least 1 hour in advance (where possible) if a lesson is cancelled due to weather.
3. If a class is cancelled by the coach due to inclement weather, the coach will offer make up class options throughout the term. If no suitable options are offered then a credit to the following term may be applied (There will be no refund for classes cancelled due to weather).

ADDITIONAL ITEMS:

1. Please disclose any health concerns (e.g., asthma, heart conditions) to coaches before enrollment.

2. The club may take photos or videos during classes, activities and performances from time to time. The photos or images taken/recorded may also be used for club promotional purposes. Please contact us if you do not want any photos/videos shown on the club’s website, social media or magazine.

3. Students are requested to arrive 5 minutes early in order for the class to run smoothly and start on time. Latecomers may not be permitted to join. 

4. Appropriate swimwear required, including goggles, swim cap. In the event of cold weather a thermal jacket is highly recommended.

5. Disruptive behavior may result in expulsion without refund.

 

By submitting this form, I agree that the relevant coaching fees will be charged to my Club account upon confirmation of my/my child's first lesson and understand the Cancellation/No Show policies as stated above.

I also acknowledge that I/my child am/is taking part in this activity at my/their own risk, and that the Club and its staff/contractors are not liable for any injury/accident to me/my child, however caused.
