Release & Liability
I certify that the above named youth is physically fit to participate in The Highlander Basketball Camp and I authorize the staff to act in their best judgment in any emergency requiring medical attention. I hereby release, indemnify, and hold harmless The Highlander Basketball Camp and Staff from and against any, and all, claims and liabilities of any and all kind and/or nature whatsoever arising from any damages and/or injuries my child may sustain as a result of participation while in camp.
If a child leaves camp for any medical reason, he or she cannot return without medical clearance from a physician. At any time, a coach can require medical clearance of a player to be able to participate or play at his/her own discretion.
Photo release statement: I also hereby consent, without further consideration or compensation, to use (full/partial) of all photos, audios or videos made of my child or family members during The Highlander Basketball Camp, for the purposes of events documentation and promotional material.
