Faith Formation Registration Form 2025-2026

Family must be registered with the parish to register for Faith Formation Classes.

If you are a new parishioner and need to register with the parish, registration forms are available at:

Forms are also located in the parish office and vestibule of the Church.

· Students in PreK-7th grade and Year 1 Confirmation: $75.00/student

* This includes Baptism and First Communion

· Confirmation Sacramental Fee (ONLY Year 2 Confirmation) - $100.00

Class Options:

Sundays; 11:00am-12:15pm (classes taught in English or in Spanish)
Wednesdays: 5:30pm-6:45pm (Classes taught in English)
Thursdays: 5:30pm-6:45pm (Classes taught in Spanish)

*There will be a limit of 16 students in each classroom. When a class fills, you will be given your second choice that you selected on your registration.

What you need to have when registering:
• Birth Certificate
• Baptismal Date and Location
• First Communion Date and Location
• Fee is $75.00 per student (Credit/Debit Card, Cash or Check in the Parish Office)

Registration opens on 3/15/2025 and closes on 6/30/2025.

After submitting this form, you will be directed to the St. James Online Giving page. Once you submit your payment of $75.00 per student, your child will be enrolled in Faith Formation classes for the 2025-2026 school year.

Parent/Guardian Faith Formation Covenant 2025-2026

Recognizing our obligation to educate our children in the ways of the Catholic faith, we hereby enter into this covenant with St. James Catholic Church Faith Formation program, to work as partners in providing for a quality program of faith formation for our children.

· Attend every class for the full time.

· Have my children arrive on time.

· Pick up my children on time with no early pick ups.

· Attend all Sunday Masses and Holy Days of Obligation.

· Submit to the office a copy of my child’s Birth Certificate, Baptismal Certificate and First Communion Certificate.

· Spend time in prayer, conversation, and support with my children in preparation for their Sacrament.

I understand that by signing below, I agree to all the terms which are listed above.

DISCLAIMER: If Birth Certificate and Sacramental Certificates (Baptism
and/or First Communion), and payment of $75.00 per student are not received, your child will go on a wait list
and seat will be given to the another student.
