Permission to Participate & Waiver

I hereby grant permission for my child (“Camper”) to participate in FC Rhythm Baseball Kids Camp (“Camp”) sponsored by Full Count Ministries and to participate in all planned program activities. I certify that the Camper is capable of participating in such activities. I understand that there is a risk of personal injury in any activity. In the event of an injury or illness affecting the Camper, I authorize the Camp or its designee to treat the Camper. In the event that reasonable attempts to contact me at the telephone number listed have been unsuccessful, or in the case of a true emergency, I hereby give my consent for any treatment deemed necessary by a licensed physician or dentist and/or the transfer of the Camper to the nearest hospital. In consideration of the permission granted to the referenced Camper to participate in Camp, I hereby release Full Count Ministries, its agents and employees, for all actions, causes of actions, damages, claims or demands which I, my heirs, executors, administrators or assigns, may have against Full Count Ministries, its employees and agents. Enrollment and participation in the Camp are voluntary and at the sole risk of the Camper and the parent(s) or guardian(s).

PHOTO RELEASE

As the parent or legal guardian, I grant Full Count Ministries my permission to take photographs and videos of my child during camp session activities. I further give permission and consent that any photographs and videos may be published and used by Full Count Ministries for publicity, illustration, advertising, social media, and web content.
