Model Release Form

Releasee: CM Play Photography LLC

(If the Model is under 18, the Parent or Legal Guardian must sign on behalf of the Minor.)

1. Consent and Authorization
The undersigned Model hereby grants permission to the Releasee to photograph, record, and/or capture digital images, video, and/or audio (collectively, “Media”) of the Model. This Media may be used for purposes including, but not limited to, advertising, promotion, portfolio use, and editorial purposes in various forms and media (e.g., print, digital, online, broadcast).

2. Representations and Warranties
The Model represents and warrants that he or she is either over the age of 18 or, if under 18, that the consent of a Parent or Legal Guardian is provided as required in Section 3 below.
The Model further represents that any provided information is accurate and that there are no restrictions or legal impediments that would prohibit the use of the Media as described.

3. Minor Consent (If Applicable)
For Models Under 18:
I, the undersigned, am the Parent or Legal Guardian of the minor named below. I confirm that I have the legal authority to grant this consent on behalf of the minor.

4. Release of Claims
The Model (or, if under 18, the Parent/Legal Guardian) hereby releases and discharges the Releasee, its employees, agents, and assigns from any and all claims, demands, or liabilities arising out of or related to the use, reproduction, or display of the Media, including any claims for defamation, invasion of privacy, or infringement of any rights.

5. Compensation
The Model acknowledges that the use of the Media is granted without expectation of monetary compensation.

6. Governing Law
This Release shall be governed by and construed in accordance with the laws of the State of Ohio. Any disputes arising under this Release shall be resolved in the appropriate courts located in Franklin County.

7. Entire Agreement
This Release constitutes the entire agreement between the parties concerning the subject matter herein and supersedes all prior discussions, agreements, or understandings of any kind. Any modifications to this Release must be in writing and signed by both parties.

8. Signatures
For Models 18 and Over:
I, the undersigned Model, have read and fully understand the terms of this Model Release and consent to the use of my Media as described above.
