We understand that surrendering a parrot is never an easy decision. Many guardians arrive here after careful thought, love, and concern for their bird’s future. This Agreement ensures that Totally Flocked Avian Rescue, Inc. (“Totally Flocked”) can act fully and lawfully in the best interest of the parrot entrusted to our care.

LEGAL SURRENDER & PERMANENT TRANSFER OF OWNERSHIP By signing this Agreement, I hereby voluntarily, permanently, and irrevocably surrender the parrot described herein to Totally Flocked, Inc. I understand and agree that:

All ownership, custody and control rights to the bird are permanently relinquished.
I certify that no other person has a right of property to the bird.
I understand that by surrendering my rights to the bird, the bird may be rehomed/adopted to an approved adopter.
I understand that once I relinquish the bird, the bird will not be returned and I have no right to reclaim the parrot at any time.
I further certify that I have read and understand the terms of this Parrot Surrender Form
I hereby certify that I am the rightful owner, keeper, caretaker, custodian of the bird(s) who is/are the subject of this Parrot Surrender Form.
Totally Flocked becomes the sole legal owner.
Totally Flocked has exclusive authority to make all decisions regarding: Veterinary care and treatment, rehabilitation, training, enrichment, adoption, placement or permanent sanctuary, and transfer to another qualified rescue or professional facility, if in the bird’s best interest.
