Hello! Thank you for contacting me regarding your social study. This social study can be used for temporary and permanent managing conservatorship or adoption.

The social study usually consists of 2-3 hours of face to face interactions with you and everyone in your home. It will consist of individual interviews, joint interviews, and family time, as well as a tour of your home. A separate visit may be requested at an additional expense, and this will be discussed on an as needed basis. My fee is ranges from $1500-1750 (fee increases for expedition) for a private adoption study which includes the time spent with your family, assessments made during our meeting, follow up paperwork and references, and travel time. I can accept Zelle, Venmo, and can accept credit cards through square which will include a 3% processing fee. Full payment is due at the time noted in the invoice. A follow up post placement visit may be necessary if the child is not yet placed in your home. This will be a seperate visit after the child is in your home for an additional fee of $300. I, as the social worker, reserve the right to increase the number of post placement visits based on the assessment of placement; however, if needed, I will inform you the justification.

Below is a list of everything needed to complete your study. Please provide me your address and phone number in a reply email at the time of scheduling. You may email me this list or provide it to me the day of your social study. Please provide the reference list now as well as complete the central registry form and FBI fingerprints before our scheduled home visit.

1) Everyone who lives in the home needs to be present during the home study. 

2) Please complete the personal history questionnaire. (can be located on my website) 

3)Please complete a budget which includes your gross AND net income.

Please print out paystubs as well and the top copy of your last tax return.

4) Provide copies insurance for health and vehicles. Please be prepared to show me the cards at the time of the meeting.

5) Animal records showing rabies vaccination. These must be current. 

6) Forward the reference link to 5 references (anyone who knows you AND the child(ren).) Additionally please forward the link to ALL adult children even if they do not live with you.  

7) Car information: Year, Make, Model; Lease or Own

8) I will need the birth parents information including names and current location if you have it.

9) Please provide copies of marriage license, decree of divorce if applicable, birth certificates of prospective adoptive/placement parents, and copies of a valid and current driver's license.

10) Please complete fingerprints and email me or screenshot a copy of the results. The website is https://uenroll.identogo.com and the code is 11 BRXN. When entering the information, you will put yourself as the recipient and then forward me the results via email. If you put me as the recipeint, I cannot guarantee receipt. You will keep the original to provide to your attorney. 

I will be asking how the child came to be with you in your home and plans for future contact with the biological parents. Please be mindful of this during our interviews in the event you do not want the subject child or your children to know all of the history. If you haven't discussed the plan for adoption/placement with them, please let me know in advance.
