A PERMISSION FORM IS REQUIRED FOR EACH ATTENDING YOUTH
We have officially booked our TTAG Youth Spring Break event (7th-12 grade) It will be at Sky Zone in Little Rock. We will leave the church on Tuesday, March 25 at 9:30am and should return before 3:00pm. The cost per student is $22.00 plus extra for lunch. Please fill out the information below to give your child permission to attend this event. You can pay the fee by using cashapp, check, or cash. Payment and Permission is due by Wednesday March 19.
Below is a waiver for Sky Zone that needs to be filled out per student as well.
