I understand that the Cooperating Fund(s) will act solely as the agent of the noted Home Fund(s) and as such, I shall be subject to the eligibility rules of said Home Fund(s) upon the transfer of contributions. I hereby release (on behalf of myself as well as on behalf of anyone claiming through me) and further discharge the Cooperating Fund(s) and their Trustees of and from all claims, demands, actions, causes of actions or suits with respect to any contributions so tranfered and for any benefits or credits which would have accured or become payable to me had I authorized this transfer of contributions. I further recognize that the transfer of contributions to the noted Home Funds may or may not ultimately prove to be the advantage of myself and/or my beneficiaries.

International Association of Bridge, Structural, and Ornamental Iron Workers Local No. 568
I authorize Local Union No. 568 of the International Association of Bridge, Structural, & Ornamental Iron Workers to represent me in collective bargaining with my present and future employers on all present and future job sites within the jursidiction of the Union and also hereby authorize and direct my employer to deduct from my wages the negotiated amounts of working dues, I.P.A.L. (Iron Workers Political Action League), and I.I.I. (Institute of the Iron Workers Industry), Savings, and any future applicable funds, and to forward the total of such monies no later than the fifteenth (15th) day of the month following the month in which money was deducted, to the Iron Workers Welfare-Pension & Working Dues Fund. These deductions shall be made from all wages earned by me while I am working in the jurisdiction of Local Union #568.

This authorization shall be irrevocable for a period of one (1) year from the date hereof or until the termination date of said Agreement, whichever occurs sooner, and I agree that this authorization shall be automatically renewed and irrevocable for successive periods of one (1) year unless revoked by written notice to the Union ten (10) days prior to the expiration of each one (1) year period, or of such applicable bargaining Agreement between the employer and the Union, whichever occurs sooner.
