    We are delighted to accompany you in preparing for this beautiful sacrament.

    If you are requesting baptism for a child from newborn through age 7, you may proceed with this form. If the child (or adult) is 8 years old or older, please contact the parish office so we can guide you through the appropriate preparation process.

    You will need to upload a copy of the child’s birth certificate as part of this request. If the child has not yet been born, please submit documentation showing the estimated due date.

    By completing this form, you are officially beginning the baptism preparation process.

    The preparation process includes:

    • Submission of the child’s birth certificate
    • Godparent forms, completed and signed by a priest
    • Certificates of completion for a baptism preparation class for both parents and godparents (completed within the last 4 years)
       

     Please note that at St. Thomas Aquinas, a baptism date and celebrant are not confirmed until all preparation steps and required documents have been completed. The date you request will be reviewed and is subject to the pastor’s approval, the availability of the baptistry, and completion of all necessary preparation requirements.

  • Godparent/Sponsor Information

  • The role of a Godparent or Sponsor

    A godparent or sponsor plays a very important role in the journey of someone to be baptized. A godparent or sponsor must be good examples of the Catholic faith and sources of support for both the child and the parents.
  • Per Canon Law 874 the godparent must meet ALL of the following requirements to be a godparent or sponsor. They will be asked to testify to the following requirements to take on the role of godparent/sponsor:

    A godparent/sponsor must be fully initiated into the Catholic Church. Please indicate whether you know that the godparent has received the three sacraments of initiation:
  • Regarding the 6th Commandment, a godparent/sponsor must be in good standing with the Catholic Church.

    •  If married, he/she must be married in the Catholic Church and living in accord with Church teaching. Catholics who were married by a Justice of the Peace, wedding officiant, or at another Christian church without special permission do NOT meet the requirements of the role of godparent/sponsor.
    • If single, he/she must be living according to Church teaching. Catholics who are cohabiting (living with a boyfriend or girlfriend) do NOT meet the requirements of the role of godparent/sponsor.
