As the parent/guardian of the above-named participant(s), I hereby give my constent for emergency medical care prescribed by a daily licensed Doctor of Medicine or Dr. of Dentistry. This care may be given under whatever conditions are necessary to preserve life, limb, or well-being of my dependant.

Although the safety of all sport activities is the primary concern, indoor sport activities at Skybacher Ministries Inc. may cause injuries and/or death. During the camp day, we take photographs of activities involving campers to share the positive vibe and updates.
