By submitting this application to become a lash model, I acknowledge and agree to the following terms:*

I understand and accept that this session is intended to test and experiment new lash techniques, and I am open to the final outcome. I consent to being filmed and photographed during this session and authorize the lash artist to use this content for social media purposes. I understand that I may receive either a free or discounted lash set based on my social media following and engagement. Final selection and compensation are at sole discretion of the lash artist (Refinedbymx). I agree to be available for a total duration of 4-5 hours to accommodate the lash application process and the creation of social media content. I agree to arrive on time and understand that my attire must be clean and camera ready. I understand that the lash artist will have full control over the design and style of my lash set as the session is intended to explore and test new techniques. I understand that I must be 18 years or older and that I will be asked to read and sign all forms regarding before and aftercare, consent and some personal health information. I understand that I will be asked to pay a deposit to ensure my presence. I agree that weekly appointments are recommended but not included. If a second appointment with Refinedbymx is desired, Additional charges will apply.
