    • * I understand that in the case of injury or illness, every effort will be made to contact me in a medical emergency.  In the event I cannot be reached, I give permission to parish staff to secure all proper and necessary treatment for my child(ren).  I understand that no liability is assumed by Our Lady of Perpetual Help Parish or the Diocese of Trento for claims that may arise.  

      *  Parental Consent for Medical Care: In case of an emergency, I give permission for my child to receive emergency medical treatment and, if necessary, be transported to the nearest appropriate medical facility.

      * I give permission for this information to be shared with our catechists.

      POLICIES & PROCEDURES

      1. ARRIVAL- It is very important for students to arrive on time for class. Please be aware, doors will be locked 10 minutes after classes begin. All students are to come to class prepared with work at home assignments.
      2. ATTENDANCE
        1. 1st - 6th Grade - Only 1 unexcused absence will be allowed for our Family sessions and only 2 at home assignments can be missed.  If more than 2 at home assignments are missed and not made up then that will be marked as an unexcused absence. Absences due to a child’s illness are considered excused. Excessive absences without communication to the PREP office will result in repeating the year. Children who accumulate more than two unexcused absences will not receive credit for the completion of the year. This may affect entry into sacrament preparation processes. 
        2. 7th & 8th Grade Confirmation - Only 2 unexcused absences will be allowed per year. Students who accumulate more than two unexcused absences will not receive credit for the completion of the year. This may affect entry into sacrament preparation processes. Absences due to a student’s illness are considered excused. Attendendance is required at all Sacramental Retreats. 
      3. TARDINESS- Any student consistently arriving more than 10 minutes late without communication to the PREP office will be marked absent.
      4. SECURITY-  For the safety of your children, all doors, including the front door, will remain locked 10 minutes after classes begin. Therefore, please make every effort to drop off your child on-time.  
      5. HOMEWORK- *Home chapters are a vital component of the partnership for family formation*   We are sensitive to the fact that students get a great deal of homework in school and with that in mind our Work at Home assignments will reflect that. It is expected that together you will go through the lesson with your child(ren) and that they will complete the given assignment for each week. Completion of these lessons is necessary for their Faith Formation and DO count toward attendance. Assignments are to be returned the following Family Session.
      6. MASS ATTENDANCE- 
        All families should be committed to attend weekly Mass. Mass attendance is necessary for your child to be fully engaged in the development of their faith. It is also essential for successful participation in classroom discussion as well as fully understanding lesson content.
      7. STUDENT RESPONSIBILITIES:
        1. Be punctual and go directly to your assigned class
        2. Be attentive and participate during class
        3. Be respectful to teachers and classmates
        4. Complete assignments
        5. No food or drink, except water, is allowed inside the building
        6. Cell phones are to be turned off during class and out of sight. (If students continue to use their cell phones during class, the phone will be taken from them and their parent will have to come to Mrs. Obuchowski to retrieve their phone at the end of the session)
      8. PARENT RESPONSIBILITIES
        1. Primary responsibility- commit to…
          1. Drop-off child(ren) to the student only session on the 1st Wednesday monthly.  This is subject to change for Christmas, New Year's and Easter.
          2. Attend the Family session on the 3rd Wednesday monthly.
          3. Weekly at-home lessons w/ child(ren)
          4. Weekly worship at Mass
        2. Strive to be punctual for drop off of your child
        3. Instill a positive attitude in your child
        4. Share your own faith experiences with your child
        5. Attend scheduled parent meetings
        6. Stewardship- Contribute your time/talent/treasure as God invites you to. 
      9. TUITION
        Tuition is to be paid in full by March 1, 2026.
