  • Dear respondents,

    We are a group of undergraduate psychology students from the Faculty of Social Science and Liberal Arts, UCSI University. We are currently conducting a research to analyse the Depression, Anxiety and Stress Scale (DASS) for our course, SP308 Psychological Testing and Assessment. This questionnaire allows respondent to understand their degree of experiencing symptoms related to depression, anxiety, and stress.

    This questionnaire consists of 3 sections:
    Section A: Demographic Profile
    Section B: Depression, Anxiety and Stress Scale (DASS) Questionnaire
    Section C: Results of the assessment

    The responses will be used for research purposes only. Hence, we greatly appreciate your honest responses.

  • In this section, we will be collecting your personal information.
    All data collected will be kept confidential.

  • Section B: Depression, Anxiety and Stress Scale (DASS) Questionnaire

  • Please read each statement and choose one number between 0, 1, 2 or 3 which indicates how much the statement applied to you over the past week. There are no right or wrong answers. Do not spend too much time on any statement.

    The rating scale is as follows:
    0 - Did not apply to me at all
    1 - Applied to me to some degree, or some of the time
    2 - Applied to me to a considerable degree, or a good part of time
    3 - Applied to me very much, or most of the time

  • Interpretation of total distress scores:

    - Normal: 0-32
    - Mild: 33-39
    - Moderate: 40-49
    - Severe: 50-57
    - Extremely Severe: 58 and above

  • Interpretation of total depression, anxiety and stress scores:
