I, the undersigned, request that Cheryl Colega provides me proactive educational and coaching assistance to facilitate my better understanding of health & nutrition in order to bring greater health vitality, energy, and balance into my life.
 
In participating in this educational health advocacy process, it is inclusive of my tendering any body part, fluid, or tissue, needed to audit my own health physiology by way of any physical, mechanical, optical, electrical, photonic or any other method, done at my sole discretion, under retained right, and in private association with Cheryl Colega.
 
I further agree compensation for such service discussed or outlined elsewhere is appropriate and that this constitutes a private contractual matter between myself and Cheryl Colega and is not a public arrangement, and where these things and my health are private matters, they shall be kept private.
 
I understand that Cheryl Colega has received non-accredited training in Microscope & Nutrition Technology, has a Pharmacist Degree from Duquesne University, has Health Coach certification from Institute of Integrative Nutrition, but is not offering nor providing service to me in any public capacity or under any government program, insurance, license, registration, certification, or regulation.
I understand that the requested service under this agreement is not a substitute for regular medical or other licensed care and does not provide any diagnosis, prescription, or treatment for any medical or government defined health condition wherein only a government certified, registered, or licensed professional might be deemed competent or authorized to address such issues. I acknowledge that I alone bear full responsibility for my own health and any actions taken related thereto.
 
AMENDMENT IX, U.S. CONSTITUTION
“The enumeration in the Constitution, of certain rights, shall not be
construed to deny or disparage others retained by the People.”
 
Under the 9th Amendment to the Constitution of the United States of America, I retain the right to freedom of body autonomy and choice in health care and education. This includes the right to choose my diet, to obtain, purchase, use or deny the use of, any therapy, regimen, modality, remedy, device or product recommended by any therapist, doctor, practitioner, consultant, coach, health advocate or other individual I might engage, whether deemed by anyone else to be right or wrong, accepted or not. The enumeration in this declaration shall not be construed to deny or disparage other rights retained by me, or my right to amend this declaration at any time.
 
CONSTRUCTIVE NOTICE
 
Notice is hereby given to any person who receives a copy of this Declaration and who, acting under the color of law, intentionally interferes with the free exercise of the rights retained by me, that they may be in violation of my private property rights under contract as well as my civil and constitutionally protected rights (see Title 42, U.S.C. 1983 seq. and Title 18, Section 241).
