Get On Board Holiday Camp 2025 Consent & Waiver
I hereby declare that I am the legal parentguardian of the above-named child and consent to the child's participation in the activities that are described to me in registration process. I understand that activities of the kind described may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities. It is the responsibility of parents and carers to ensure children are equipt with protective equipment i.e helmet and pads. I agree that no legal action will occur against Get On Board and to disclose any medical information to Emily including ADHD, Austism.
If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to staff to deliver first aid.
I, the parent/guardian, hereby agree and declare that I understand the scope of the skateboarding lessons and I consent to the participation of the above-named child to these activities.
I consent to any photos/video being taken of my child or will inform Emily if this is something I do not consent to.
