The American Heart Association's Cardiovascular-Kidney-Metabolic Health Initiative
The Cardiovascular-Kidney-Metabolic (CKM) Health Initiative, funded by Founding Sponsors Novo Nordisk and Boehringer Ingelheim, Supporting Sponsors Novartis Pharmaceuticals Corporation and Bayer, and Champion Sponsor DaVita, aims to reach 265,000 patients across 150 health care organizations. The initiative seeks to integrate CKM health metrics into existing data registries, develop a scalable, patient-centered certification framework and establish a CKM health recognition program for providers and health systems. Additionally, the initiative focuses on increasing screening for social determinants of health and incorporating community health workers and social workers into care teams. Interested in applying?
