I have read and understand the NLEA Provincial Official: Judge application requirements and information, and hereby submit the above information as my application for said program. I hereby declare that the above information, to the best of my knowledge, is true and complete.

If selected, I understand that my name will be added to the NLEA approved officials list, but no judging opportunites are guaranteed at any time. I also understand that all contract details, including payments, are to be negotiated with the competition organizer(s) directly (i.e. the NLEA have no financial obligation for this role at any time).

Furthermore, I acknowledge that my application will be rejected if payment is not received at the same time of application submission.
