Self Evaluation

The first step to you re-entry plan is to make a list of services you will need. A successful transition depends on making sure your basic needs are met. Since no one knows your needs better than you do, it is important to be very clear about what you will need and what resources are available to you. Here are some questions to help you get ready for your transition from incarceration to the community.

3) What services are most important for you to be successful? Do you know how to access these services?

5) Who can you rely on for support to suceed in these plans?
6) What are your favorite activities to do?
7) Would you like to discuss any additional items with your Discharge Planner?
