I, the undersigned parent or legal guardian of the above-named player, hereby give permission for my child to participate in the Liga de San Lorenzo. I acknowledge that participation in soccer activities involves inherent risks, including but not limited to injury, accidents, and physical exertion.

By signing this waiver, I agree to assume full responsibility for any risks associated with my child's participation in the camp. I hereby release and hold harmless Liga de San Lorenzo, its organizers, staff, volunteers, and any affiliated parties from any and all claims, damages, injuries, or losses arising out of my child’s participation in the soccer league.

In case of emergency, I authorize the league staff to seek medical attention for my child if necessary. I understand that I will be notified as soon as possible in such an event.

I also acknowledge that I have provided accurate medical and emergency contact information and will update the camp immediately if there are any changes.

PLEASE HAVE YOUR PLAYER BRING WATER, BALL, SHIN GAURDS. Payment will be due once team registration is completed. Team coaches will be responsible for scheduling their own team practices, uniforms, and rosters. Schedules are final.
