RCCXH Productions Inc., hereafter "RCCXH"

Full Legal Name (from above), hereafter the "Performer"

The following is understood by Performer about RCCXH:
1. RCCXH is an adult entertainment company, marketing sexually explicit content that includes, but is not limited to, images, videos, audio, other digital content, hereafter called (the "Content").
2. RCCXH will use the attached government identification to ascertain, by examining the validly issued government identification bearing a recent, clear and recognizable photograph of the Performer (with no alterations) that include a visible date-of-birth, expiration date, and the Performer's Full Legal Name, that the Performer is at least eighteen (18) years old (or over the age of majority and legal consent in the jurisdiction in which the Performer lives or resides on the date hereof).
3. The Performer guarantees that the government identification was validly issued, unexpired, and was not altered in any way.
4. RCCXH understands that the Performer may appear in created Content under an stage/artist/model/performer name. RCCXH agrees to not disclose the legal/real name, or any other contact details, to any third party without the explicit written consent of Performer or the legal requirement to do so.
5. The Performer may discover the legal names of individuals/companies/etc. associated with RCCXH; the Performer is prohibited from disclosing these legal/real names, or any other contact details, to any third party without the explicit written consent of RCCXH.
It is also understood that in addition to these guarantees there is a contract to be signed, which is part of this intake process (which is where the pay structure is outlined).
By clicking Submit the Performer is certifiying that:
- The person submitting/filling in the form is the Performer themself, not someone doing it on someone's behalf.
- The Performer fully understands that they are submitting the form, and all attachments, to RCCXH with the intent to become an adult performer.
