By participating/exhibiting at DCF Family Fun Walk/Run, produced by Disabled Children’s Fund, DCF, and registering to attend as a participant or a vendor at the MidAtlantic Community Church, MACC, campus grounds in Gambrills, MD, I agree to the following:

1. To indemnify and hold harmless, DCF Family Fun Walk/Run, Disabled Children’s
Fund, DCF, the MACC and its agents associated with DCF Family Fun Walk/Run,
all sponsoring organizations and their elected officials, directors, employees,
representatives and volunteers from any responsibility, injury, property damage,
loss, personal liability, or claims of loss, damage or expenses that may occur or
relating to the participation/attending within DCF Family Fun Walk/Run. DCF
Family Fun Walk/Run or the MACC is not responsible for any injury sustained by
exhibitors, patrons, dogs or guests.

2. While attending DCF Family Fun Walk/Run with my dog(s) and should any injury
occur to any person, other dog or property as a result of the behavior of, which
includes biting by, my dog(s) at DCF Family Fun Walk/Run whether expected or
unexpected, regardless of the stimulus for the behavior (i.e., dog’s spontaneous
conduct, the conduct of another dog, the conduct of any person or thing, or a
combination of any of these), I will be solely liable and be held responsible for
any injury or property damage that results from (including reasonable attorney’s
fees) and that I will indemnify, hold harmless, protect and defend Family Fun
Walk/Run, MACC, all sponsoring organizations and their elected officials, directors, employees, representatives and volunteers, for any and all claims of liability and damages from any injury, property damage, loss, claim, or expense resulting from the behavior of my dog.

3. To report any and all such injuries immediately to DCF Family Fun Walk/Run
Management.

4. That bringing my dog into DCF Family Fun Walk/Run I understand that I am
doing so at my sole risk, cost and expense and any harm that comes to the dog(s) be solely my responsibility; and that I will be solely responsible for cleaning up any messes made by my dog(s).

5. That my dog(s) is healthy, free from parasites & up to date on all shots including Rabies and Distemper vaccinations.

6. I understand if my dog(s) acts in any way considered aggressive or dangerous, I
will have to leave the event and will not be entitled to any refund.

In consideration of the acceptance of my entry. I hereby for my heirs, executors and administrators assume any and all risks which might be associated with the DCF Family Fun Walk/Run. I waive and release any and all right and claims of any nature for damages which I may have against the organizers and all others connected with this event, their representatives, successors and assign for any and all injuries or damages of any kind whatsoever suffered by me, my minor children or my dog(s) as a result of taking part in the walk and any related activities.

I give my permission for photographs taken at the event in which my image or the
image of any of my minor children and dog(s) appears to be used for promotional and/or advertising purposes by DCF without compensation to me or my minor children.

I further acknowledge that by my signature, I have read and consent to the DCF Family Fun Walk/Run Liability Waiver.

 

1. Dog Owners/ Handlers must consent to the liability waiver and to the rules set
forth for each dog participating in the DCF Family Fun Walk/Run.

2. Only one dog allowed per owner/handler, so each dog must have his/her own
human. No sharing! But a dog may bring as many humans as they want!

3. Dogs must be well mannered. All dogs MUST be kept on a leash (no longer than
6 ft.) or in an enclosure or enclosed in a dog-friendly enclosure (strollers, etc).
Please NO retractable leashes! Each dog must wear a secure collar or harness. It
is recommended your dog be wearing a form of proper ID attached.

4. For the safety of both our human and canine attendees, your dog must be up-to-
date on all its vaccinations.

5. NO female dogs in heat!

6. When bringing your dog to DCF Family Fun Walk/Run, please use common sense! If your dog gets nervous around large crowds or does not interact well with people or other dogs, is not a social, please leave them home.

7. ALL dog owners bringing dogs to DCF Family Fun Walk/Run will be asked to sign
a liability waiver with registration for the DCF Family Fun Walk/Run if not already
done with on-line registration.

8. If your dog should have an accident, clean up stations are located throughout
the grounds. It is your responsibility to clean up after your dog!! Failure to do so
can result in immediate removal from the event.

I further acknowledge that by my signature, I have read and consent to the DCF Family Fun Walk/Run Rules for Participating for Dogs & Dog Owners.
