Incident Details
- Date of Incident: When the issue occurred
- Time of Incident: Be specific (shift time, break time, etc.)
- Location of Incident: Where it happened (e.g., gate number, aircraft, break room, etc.)
- People Involved: List names, titles, and their role in the incident (Witnesses, Supervisors, MODs, etc.)
Description of the Incident
- Provide a factual and detailed account of what happened.
- Stick to what you saw, heard, and experienced—avoid opinions or assumptions.
- If disciplinary action was taken, note what was said or provided in writing by management.
Policy or Contract Violation (If Known)
- Reference specific sections of the Collective Bargaining Agreement (CBA), company policies, or labor laws that were violated.
- Mention Article 3.C (or other applicable articles) if discipline was unjust.
- Cite the Seven Standards of Just Cause (if relevant).
Witness Statements (If Available)
- Include any witness names and contact info.
- If a witness is willing to provide a statement, include it or reference where the witness can be found.
Supporting Evidence
- Documentation: Attach emails, reports, company policies, or any other written proof.
- Photos/Videos (if applicable): If you have relevant visual evidence.
- Past Precedents: If other employees were treated differently for the same issue, note that.
Desired Resolution
- Clearly state what action you want (e.g., removal of discipline, back pay, policy clarification, manager training, etc.).
