General Program Activities Authorisation Form

The program outlines the activities and theme of what is happening during that day at Vacation Care, however we have time throughout the day to have spontaneous activities and unplanned activities including programs and workshops onsite, activities suggested by children, free play, and playing outdoors in nature. This authorisation form allows your child to participate in these activities each day during Vacation Care.

A helmet must be worn while riding.I acknowledge that EHOOSH is not liable for damages, breakage, or loss of personal belongings, including toys, wheels, equipment, or devices.

Base Zero Rock Climbing Onsite Waiver Form - Wednesday 23rd April

The Base Zero Rock Climbing Wall will be held onsite. Children will be supervised by qualified instructors and EHOOSH educators at all times. They will be harnessed at all times when climbing. If you have booked your child for this day, please fill in the details below before moving on.

Top Ryde Movies Excursion (Dog Man) Authorisation Form - Monday 28th April

The excursion will be held at Top Ryde Event Cinemas. Children will be away from the service from 10:30am and return by 2:00pm. The ratio for this excursion is one educator to eight children and the anticipated number for this excursion is 60 children with nine educators. Children will be supervised by EHOOSH educators at all times. A risk assessment has been completed and is available at the service upon request. If you have booked your child for this excursion, please fill in the details below before moving on.

I give permission for my child to attend the excursion to Top Ryde Event Cinemas on Monday 28th April 2025.I acknowledge that Glenorie coaches private bus might not be fitted with seat belts.Arrive by 10:25am, the bus will not wait for late families as we have a scheduled booking.I acknowledge my child will take their water bottle, lunch, receive popcorn provided by Event Cinemas, and are not allowed to purchase any lollies or snacks.EHOOSH has hired a private cinema for viewing, the movie is 'Dog Man' and is rated PG.I acknowledge my child will be away from the service from 10:25am and return by 1:35pm. Lunch will be consumed at the site.I acknowledge that if my child arrives at the service without an EHOOSH t-shirt and hat they will be given a new one and my account will be charged.
I hereby give permission for my child to participate in programmed activities and attend the excursion to Top Ryde Event Cinemas as nominated on the program on Monday 28th April 2025, as well as participate in all walking required to attend the excursion of the premises of the service.*
