Appointment Request Form

I cannot wait to meet you and bring your vision to life!!
A few things you should know before filling out this form:

• You must be 18 years or older to be tattooed. Valid non-expired state ID or passport will be required the day of your appointment.

• All designs will be presented in person the day of your appointment.

• PRICING TIER:

•I do not do hourly rates, I am a bit of a perfectionist and price per project. A total price will be provided before our first session during the in person consultation. I will not charge you more because I am particular and want to ensure that I can provide the best quality possible.

• Deposit is $100 and required during the in person consultation. The deposit is non refundable, for any reason but can be rescheduled if the artist is notified within 48 hours of the appointment.

•The deposit is applied to the total amount of your tattoo. On the final last session it will get deducted from the total amount. Deposits can be rolled to a new project if you want to book a new project right away.

•In person consultations are preferred and are a way that we can talk and meet, make sure that were going to have a great time working together and go over any last minute deatils for your tattoo vision!!
