MEDICAL CONSENT

In the event of an emergency, I authorize the Diocese and its representatives to obtain medical treatment for me as deemed necessary. I understand that the Diocese is not responsible for any medical costs incurred due to illness or injury and that I am solely responsible for all medical expenses.

RELEASE OF LIABILITY AND INDEMNIFICATION

 
I, the undersigned participant, acknowledge that my participation in the Vocations Discernment Event at Knott’s Berry Farm is entirely voluntary. I understand that this event involves travel, participation in amusement park activities, and other physical activities that may carry inherent risks, including but not limited to injury, illness, or damage to personal property.

By signing this agreement, I hereby release, discharge, and hold harmless The Roman Catholic Bishop of San Bernardino, a Corporation Sole, the Diocese of San Bernardino, its affiliated entities, officers, employees, volunteers, agents, and representatives (collectively referred to as “the Diocese”) from any and all liability, claims, demands, actions, causes of action, costs, expenses, damages, or liabilities of any kind arising out of or related to my participation in this event.

 
This waiver includes, but is not limited to:

• Any accident, injury, illness, or property loss occurring before, during, or after the event.

• Any exposure to communicable diseases, including but not limited to COVID-19.

• Any actions, omissions, or negligence of third parties, including but not limited to Knott’s Berry Farm staff, patrons, or other attendees.

• Any transportation-related incidents, including travel to and from the event.

 
I understand that this agreement is legally binding upon me, my heirs, successors, and assigns, and that I am waiving legal rights by signing it.
