CONSENT AND RELEASE OF PERSONAL INFORMATION

1.               I, the undersigned, hereby give Science Fair Foundation of British Columbia (“FOUNDATION”) consent to take, retain and publish photograph(s) and/or video recording(s), as the case may be, of my name, likeness, poses, acts and appearances, science fair projects or other materials prepared, developed or created by me, and sound recordings that are made of my voice (collectively referred as the “Personal Information”), and use such Personal Information for any purposes in connection with promoting the Foundation and/or science fair programs that the Foundation organises, exhibits and/or participates in from time to time, which may include academic, educational, advertising, promotion, marketing, artistic and exhibition purposes (the “Purposes”). In connection with such Purposes, I agree that FOUNDATION may:

     (a)   crop, edit, alter, modify or create derivative works using the Personal Information and combine such Personal Information with other images, text, photographs, video or sound recordings and graphics, without restriction;

     (b)   reproduce, post, distribute, publish, display, broadcast and/or make the Personal Information available to the public on or over the Internet (including social media sites and third party websites), or otherwise reproduce, display or publish the Personal Information, in whole or in part, in all media and formats, whether now known or here after devised; and

     (c)   disclose and release the Personal Information and contact information provided by me to third party award sponsors, for the Purposes, as well as to facilitate the receipt and administration of any award that I receive.

 

2.               I acknowledge and agree that the Personal Information is being collected pursuant to applicable privacy laws for the Purposes. For greater certainty, I consent to the use or display of my name in connection with the permitted use of the Personal Information by FOUNDATION described herein. I consent to the Personal Information, being stored, accessed or disclosed outside of Canada (if applicable, for example on a third party server that is outside of Canada).

 

3.               In consideration for the opportunity for me to participate in the event described below, I hereby covenant and agree to waive and release FOUNDATION, its directors, staff, officers, employees and agents, from any and all claims, demands, damages and proceedings arising from or relating to any use by FOUNDATION of the Personal Information, including any statutory or common law privacy rights relating to the Personal Information. Further, I hereby covenant and agree that FOUNDATION is not liable for any losses or damages, whether direct, indirect, consequential, incidental or special, arising from any use of the Personal Information, whether by FOUNDATION or any person (including third parties). This Section 3 shall survive any termination or expiry of this Consent.

 

4.                This Consent is valid with respect to my participation in the 2025 Science Fair Program.

Consent by Parent or Guardian (if the participant is under 19 years of age)

I represent and warrant to FOUNDATION that I am the parent or legal guardian of the minor child named below. I agree and acknowledge that I am required to supervise the minor child named below so he or she does not cause harm to others, to themselves or to property. I, as the parent or legal guardian, am responsible for the actions of the minor child named below. I confirm that I have read and understood this document prior to signing it and am aware that by signing this consent I am giving permission to FOUNDATION to use the Personal Information for the Purposes. On behalf of such minor child, I hereby agree to the terms of the consent and waiver set out above.
