Talent Release Form

I hereby assign toProducer* (Producer) and Greater Northshire Access Television, Inc. (operating as: GNAT-TV) all rights worldwide and in perpetuity to the recording and/or taping of my appearance by means of the videotape, SD card capture, still photography and audio record made on this date, and I hereby further authorize the reproduction, copyright, sales, exhibition, cablecast, webcast and/or distribution of said videotape by Greater Northshire Access Television, Inc. or its producer(s), agent(s) or assignee(s) without limitation. I understand that permission allows GNAT-TV, its producer(s), agent(s), or assignee(s) to distribute the production and/or excerpts from the production, and I understand distribution may include broadcast, video contests, internet, other access stations, print publicity and any distributions available to this producer and GNAT-TV. I further agree to release, discharge and indemnify GNAT-TV, including its representatives, producers, agents or assignees, from any legal proceedings or claims that may arise in relation to the conditions stated above.
