I hereby grant permission for my child, to participate in minor football activities organized by Sylvan Lake Minor Football Association (SLMFA) including acknowledgement and consent that their image/likeness may be used in marketing or promotional materials.
I understand my child will be required to participate in fundraising efforts for the Bears and/or Sylvan Lake Minor Football including the sale of the annual Football Discount Cards.
I understand my child's equipment deposit will be returned at the end of the season when equipment is returned and in good condition. Please note: Any outstanding items or cash balances owing from previous seasons must be cleared with the respective teams prior to the start of the season.
I agree to complete the Code of Conduct and Medical Release form and understand that my child will be unable to participate in SLMFA activities until they are received by the team representatives at the beginning of the first in-person session.
