    1. I understand that the massage service offered is intended to introduce the Philippines' "Hilot" massage experience and is not a substitute for medical examination, diagnosis, or treatment.

    2. I understand the risks associated with massage therapy include, but are not limited to:

      • Superficial bruising
      • Short-term muscle soreness
      • Exacerbation of undiscovered injury

    3. I do not have any medical conditions, injuries, or illnesses that would prevent me from safely receiving a massage.

    4. I will inform the massage therapist of any discomfort, pain, or concerns during the session.

    5. I am not experiencing any contagious conditions or infections.

    6. I authorize the use of oil and ointments to my body.

    7. I understand that arriving late may result in a shorter session or cancellation without rescheduling.

    8. I understand that I or the massage therapist may terminate the session at any time.

    9. I further understand that massage therapy is not a substitute for a medical examination or treatment, and that I should see a physician or other qualified health specialist for any mental or physical ailment of which I am aware. I understand that massage therapists do not diagnose illness or disease, and nothing said during the treatment should be construed as such. My consent is informed and voluntary and I understand that I may withdraw my consent at any time except for actions already taken.

    I acknowledge that:

    1. I will release the massage therapist and business from all liability for any harm that may unintentionally result from this treatment.

    2. I will not hold my massage therapist responsible for any pain or discomfort I experience during or after the session.
