BANKING INFORMATION REQUEST

Wuskwi Sipihk First Nation requests that all members provide their banking information to ensure the timely, efficient, and secure payment of the Per Capita Distribution ("PCD") under the Agricultural Benefits Settlement, which was approved by vote on January 13, 2025.

THERE ARE THREE WAYS TO SUBMIT YOUR INFORMATION & SUPPORTING DOCUMENTS:

1. Complete and submit this form using the online submission option below.

2. Submit the attached form along with your ID by scanning to the email: support@wuskwisipihk.com

3. You can also hand deliver this form to the band office during regular business hours, Attn: Bev Ready

REMINDER:

• If these forms are not completed, the PCD cannot be issued to members. Supporting documents must be attached and submitted to ensure payments are issued as soon as possible.
• Please be sure to enclose a copy of the appropriate identification and a void cheque or banking information as required. The Void Cheque or Direct Deposit Form must reflect the members' name. It may not be deposited into someone else's account.
• A member may request a cheque, but it should be noted that the safest and fastest way to receive a PCD is by direct deposit. We strongly encourage every member to request a direct deposit. The processing time for a cheque will be between 3-5 weeks. A large cheque could face further holds, security concerns and even fees if cashed at certain institutions.
• For members living outside of Canada, International Wire Transfer information from their bank is required. This includes Bank name, bank address and SWIFT CODE/ABA or IBAN #. Please include beneficiaries name, physical address (no PO Box) and bank account number.

DISCHARGE, RELEASE & INDEMNITY:

1. I do not require, nor have I received, the services of a translator regarding this Payment Instruction, Discharge, Release and Indemnity.

2. I am executing this Payment Instruction, Discharge, Release and Indemnity for valuable consideration and of my own free will and volition.

3. I am executing this Payment Instruction, Discharge, Release and Indemnity conscientiously believing all facts set out herein to be true.

NOW THEREFORE,

4. I hereby agree that, upon my receipt of the Payment consistent with these instructions, | grant to Wuskwi Sipihk First Nation, Royal Trust Corporation of Canada and the Government of Canada a full, final and complete discharge with respect to the payment and I release Wuskwi Sipihk First Nation, Royal Trust Corporation of Canada and the Government of Canada from any further obligation to account to me or make a distribution to me in connection with the Payment.

5. I hereby agree to indemnify Wuskwi Sipihk First Nation and Royal Trust Corporation of Canada, including the directors, officers, employees, and agents of Royal Trust Corporation of Canada (collectively the "Indemnified Parties"), and save them harmless from any and all liability, losses, damages, costs (judicial or not), charges and other expenses of any kind or nature arising at any time directly or indirectly out of or in consequence of any action, proceeding, account, claim or demand which may be imposed upon, or incurred by, any one or more of the Indemnified Parties in relation to the making of the Payment to me and Wuskwi Sipihk First Nation acting upon the payment instructions contained herein.

6. I agree that, in accepting this Payment:

a. I am receiving a portion of the Agricultural Benefits Settlement Compensation settled to the Trust pursuant to the Agricultural Benefits Settlement Agreement with the government of Canada, which was ratified by the members of Wuskwi Sipihk First Nation on January 13, 2025, and which provides compensation relating to the Agricultural Benefits provisions being inadequately fulfilled since the Treaty 4 signing. Canada also implemented discriminatory policies that impacted the First Nation's ability to succeed in agriculture (the "Agricultural Benefits Settlement Compensation"); and

b. I am granting to the Wuskwi Sipihk First Nation First Nation and the Trustee and Canada a full and final release and complete discharge with respect to the distribution of the Payment from a portion of the Agricultural Benefits Settlement Compensation as a per capita distribution, and further release Wuskwi Sipihk First Nation, its Chief and Council, agents and employees, and the Trustee and their respective successors, heirs, executors, administrators, estate trustees and assigns, and the Government of Canada from all further accounting and responsibility in this

7. I hereby acknowledge that I have had the opportunity to seek independent legal advice with respect to my execution of this Payment Instruction, Release, Discharge and Indemnity and confirm that I have either done so or have chosen of my own free

8. I am executing this Payment Instruction, Release, Discharge, and Indemnity for valuable consideration and intend, by my signature (or digital signature), to bind myself to the terms of this Payment Instruction, Release, Discharge, and Indemnity.

9. Any part, provision, term or declaration of this Payment Instruction, Release, Discharge and Indemnity which is prohibited or which is held to be void or unenforceable shall be ineffective to the extent of such prohibition or unenforceability without invalidating the remaining parts, provisions, terms or declarations hereof.

10. I agree that (i) this Payment Instruction, Release, Discharge, and Indemnity may be delivered personally or electronically and that I am immediately and unconditionally bound upon such personal or electronic delivery, and (ii) that a faxed, scanned, or otherwise electronically delivered copy of this executed Deed shall be deemed to be an original.
