By completing this form, the registrant requests an extension for their portfolio assessment.
4. The validity period of a (re)registration may, upon request of the registrant, be extended by the registration body if a registrant is unable to meet the portfolio assessment requirements and/or participate in the practical exam for re-registration due to medical unfitness as determined by a doctor, for a period of up to three months.
5. The period for continuous practical experience until the interim review, as referred to in paragraph 5.6, may be extended by the registration body upon the registrant’s request for a period of up to three months, with the interim review date being postponed by three months, if the registrant does not meet the requirements for continuous practical experience after assessment.
6. An extension referred to in the fourth section or a postponed date for the interim review as referred to in the fifth section will be recorded on the proof of (re)registration.
7. If, after an extension as referred to in the fourth section or postponement as referred to in the fifth section, the registrant is still unable to take the exam or meet the continuous practical experience requirements due to medical unfitness, the period may, upon the registrant’s request, be extended by an additional maximum of three months.
8. The duration of the extension of the registration validity period granted under the fourth section will be deducted from the validity period of the re-registration that follows.
9.The duration of the postponement granted under the fifth section will be deducted from the period of continuous practical experience that follows.
