MedFest Screening
After signing up for an appointment time, please fill out a medical history form. A medical history form will be emailed to you.

Athletes/Parents/Caregivers who need assistance with filling out the Athlete Application will have the opportunity to receive assistance at their appointment. Assistance may not be available for those who have 11:00 AM appointments.
