Exam Fee Refund Request
* This form should only be used for cancellations after the refund deadline has passed. Please refer to the Fee Policy for details on deadlines and requirements for partial refunds. NBHWC will consider partial refunds (up to 75% of the exam fee) beyond the refund deadline in certain situations. Partial refunds will be made only in instances of severe, unavoidable, and incapacitating circumstances that occur after the refund deadline. Documentation must accompany any request for refund. To request a refund after the refund deadline, please fill out the form below as soon as you are aware of the need to cancel the examination. Requests will be reviewed as they are received. NBHWC will notify you of the refund request status within 14 business days. Requests must be received within 14 days of the close of the examination window.
