What's this agreement?
This service agreement is between the named participant and Dream Coach. This agreement has been undertaken for the purpose of providing one or more of the reasonable and necessary supports specified in the statement included, under subsection 33(2) of the NDIS Act, in the participant’s NDIS plan.This agreement will take effect from the date it is signed for a period of 12 months (or from the start date of access to services if signed in advance of service provision).
