HEALTH COACHING CONSENT & DISCLAIMER

I, the undersigned (“Client”), understand that I am engaging in health and longevity coaching services with Kunj Patel at CRISSP Longevity Clinic in California. Although Kunj Patel is a licensed physician, these services are provided solely for educational and wellness purposes and do not constitute medical diagnosis, treatment, or prescriptions.

I acknowledge that no physician-patient relationship is established by participating in these coaching sessions. I understand that Kunj Patel’s role is limited to providing general wellness guidance in the realm of longevity, healthy living, and lifestyle modification.

• Kunj Patel will not conduct physical examinations, order diagnostic tests, or prescribe medications under this agreement.

• Coaching recommendations may include dietary guidance, exercise suggestions, stress-management techniques, and other wellness strategies.

• I understand that any recommendation made is not a substitute for professional medical advice.

• I acknowledge that the information shared is not intended to replace or contradict any advice provided by my personal healthcare providers.

• I agree to consult with my own physician or other qualified healthcare professional regarding any specific health condition or concern before making major lifestyle changes.

• I participate voluntarily and understand there may be risks associated with lifestyle changes (e.g., exercise, dietary adjustments).

• I assume full responsibility for any potential consequences of my participation, including any harm or injury arising from my decisions.

Information shared during coaching sessions will be treated as confidential to the extent permitted by law.
CRISSP Longevity Clinic will comply with all applicable federal and state privacy regulations, including HIPAA (where applicable).

I acknowledge that results from health and longevity coaching cannot be guaranteed. Individual outcomes depend on many factors, including adherence to recommended changes and personal circumstances.

This consent form is intended to comply with laws and regulations in all 50 states, including California. If any provision is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

In consideration of receiving health coaching services, I agree to release and hold harmless Kunj Patel, CRISSP Longevity Clinic, and any employees or affiliates from any claims or liability arising from or related to my participation in these sessions.

By signing below, I acknowledge that I have read and understand this consent form. I agree to the terms and fully and voluntarily consent to participate in the health coaching services offered by Kunj Patel.
