First State Flag Football League (FSFFL) Waiver & Release of Liability
1. Assumption of Risk & Release of Liability
I, the undersigned participant (or parent/guardian if the participant is under 18), acknowledge that participation in First State Flag Football League (FSFFL) involves inherent risks, including but not limited to: collisions, sprains, fractures, concussions, muscle strains, and other potential injuries. I understand that these risks may occur due to my actions, the actions of other participants, field conditions, equipment, or other factors.
I voluntarily assume all risks associated with participation in FSFFL and hereby waive, release, and discharge FSFFL, its owners, staff, referees, sponsors, volunteers, and facility owners from any claims, liabilities, or expenses resulting from my participation, including injuries, medical expenses, or damages, whether caused by negligence or otherwise.
2. Medical Authorization
I certify that I am physically fit to participate in FSFFL and do not have any medical conditions that would prevent me from safely engaging in the league.
In the event of a medical emergency, I authorize FSFFL and its representatives to seek appropriate medical care for me (or my minor child). I understand that FSFFL is not responsible for any medical treatment, costs, or related expenses.
3. Code of Conduct Agreement
I agree to abide by all sportsmanship and conduct policies of FSFFL, including but not limited to:
• Respecting referees, league staff, and fellow players.
• Refraining from unsportsmanlike behavior, violence, or verbal abuse.
• Complying with all league rules and regulations.
Failure to comply with these policies may result in ejection, suspension, or permanent banishment from FSFFL without refund.
4. Photo & Media Release
I grant FSFFL permission to use photographs, videos, or other media featuring my likeness (or that of my minor child, if applicable) for promotional purposes, including social media, website content, and marketing materials, without compensation.
5. Refund Policy Acknowledgment
I understand that FSFFL does not offer refunds after the first week of play. If I choose to withdraw before playing in a league game, I may be eligible for a refund, subject to league discretion.
6. Electronic Signature Agreement
By signing below, I acknowledge that I have read, understood, and agreed to all terms outlined in this waiver. I understand that my digital signature carries the same legal effect as a handwritten signature.
For Participants Under 18:
As the parent/legal guardian of the above-named participant, I consent to their participation in FSFFL and agree to all terms outlined in this waiver.
This waiver will be stored electronically by FSFFL for record-keeping purposes. If you do not agree to these terms, you will not be permitted to participate.
