Consent and Release Form
(For use of photographs, video and other media)
Overview: Caring by Card recruits thousands of volunteers across the United States asking them to make and send handmade cards to nursing homes that Caring by Card sponsors and partners with. To date, Caring by Card has sponsored at least one (1) nursing home in all fifty (50) states.
Purpose: Caring by Card aims to uplift the lives of the elderly, disabled and sick, one card at a time. Many nursing home residents are bed bound and do not have family or friends that visit them. Caring by Card is committed to uplifting these people through the simple act of receiving a homemade card from someone who cares. Caring by Card wishes to document and share the impact of such homemade cards on nursing home residents by posting photos on its Internet homepage located at https://www.caringbycard.com/ (the “Website”) or Caring by Card’s social media (i.e., Facebook, Instagram, etc.) (“Social Media”).
Please review the following if you, a Caring by Card volunteer, would like to share photographs on Caring by Card’s Website and Social Media. For the avoidance of doubt, participation in the posting of photographs on Caring by Card’s Website and Social Media is voluntary and entirely optional.
By signing below, I give my consent to Caring by Card and its representatives to post photographs on Caring by Card’s Website and Social Media. These materials may be used for the following purposes: (i) promoting Caring by Card's mission through marketing and communications on Caring by Card’s Website, Social Media and other public-facing platforms maintained by Caring by Card and (ii) educating the public and raising awareness about Caring by Card’s efforts.
I understand that:
1. Participation is voluntary, and this consent may be revoked at any time by notifying Caring by Card in writing.
2. Caring by Card will not post photographs or other media containing identifiable features or personal details of nursing home residents or its staff and representatives to the Website or Social Media. For the avoidance of doubt, such identifiable features or personal features include photographs of the resident's face, full names or medical information.
a. A few examples of photographs or other media that Caring by Card may post to the Website or Social Media include, but are not limited to, (i) photographs of the resident’s hands holding the homemade cards and (ii) the homemade cards placed on a table, bedstand or hanging on a wall.
3.The photographs, videos, or other media will be the property of Caring by Card and may be edited, reproduced and distributed as necessary to fulfill Caring by Card's mission.
4. No financial compensation will be provided for the use of these materials.
