The following information is required to register for the yard sale. This information will not be shared on the materials (i.e map) provided to yard sale participants.

Please complete the following information which will be included on the map for convenience for yard sale shoppers. 

TO COMPLETE YOUR REGISTRATION: 

1.  Complete this form, including hitting the SUBMIT button at the bottom of this page. If you do not hit SUBMIT you will not be registered for the yard sale.

2. Send payment to LVPCA.  LVPCA members pay $15, non-members are $20.            There are 3 convenient ways to pay:

a. Zelle to treasurer@lavistapark.org 

b. Venmo to @LVP-Treasurer

c. Drop a check in the secure mailbox at the park entrance on Brookforest.
