41SIX Beach Volleyball Clinics

Our Beach Clinics will run during the months of May and June right after school down at Ashbridges Bay. Clinics will run Tuesday and Thursday from 3:45 pm to 5:45 pm. Groups will be broken up in age and ability. Clinics are open to athletes that are completing Grades 5-11. Cost $425+HST for 12 Sessions.

TRIPLE CROWN VOLLEYBALL LTD AND 41SIX VOLLEYBALL CLUB CLINIC WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in the Triple Crown Volleyball Ltd and 41SIX Volleyball Club Clinic, I, the undersigned participant or legal guardian, hereby acknowledge and agree to the following terms and conditions:

Assumption of Risk: I am aware that participating in volleyball activities involves certain inherent risks, including but not limited to, the risk of injury or illness. I voluntarily assume all such risks and understand that the Triple Crown Volleyball Ltd and 41SIX Volleyball Club organizations and their staff cannot guarantee my safety.

Medical Consent: I hereby give my consent for the Triple Crown Volleyball Ltd and 41SIX Volleyball Club staff to seek emergency medical treatment on my behalf in the event of an injury or illness during the clinic. I understand that every effort will be made to contact me or my emergency contact before such action is taken.

Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and covenant not to sue Triple Crown Volleyball Ltd, 41SIX Volleyball Club, their officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child during or in connection with participation in the volleyball clinic.

Photography Release: I grant permission for Triple Crown Volleyball Ltd and 41SIX Volleyball Club to use photographs or videos taken during the clinic for promotional and marketing purposes.

Code of Conduct: I understand that the Triple Crown Volleyball Ltd and 41SIX Volleyball Club Clinic has established rules and regulations for the safety and enjoyment of all participants. I agree to abide by these rules and follow the instructions of the clinic staff.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND VOLUNTARILY SIGN IT. I FURTHER UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING MY RIGHT TO SUE, AND ACKNOWLEDGE THAT I AM SIGNING THIS WAIVER FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE.
